A Review of Medicinal Plant Extracts for the Treatment of Antibiotic-Resistant Wounds

Abstract
Antibiotic resistance (ABR) has emerged as one of the most critical global health crises of the 21st century. The alarming pace at which pathogenic bacteria are developing resistance to commonly used antibiotics has significantly compromised the ability of healthcare systems worldwide to effectively manage infectious diseases. This issue is particularly pronounced in the context of chronic wounds, such as diabetic foot ulcers, pressure ulcers, and venous leg ulcers. These wounds are characterized by prolonged inflammation and delayed healing, and are frequently colonized by multidrug-resistant (MDR) bacterial strains, including Staphylococcus aureus, Pseudomonas aeruginosa, and Escherichia coli. The presence of such pathogens not only complicates the clinical management of these wounds but also increases the risk of systemic infections, morbidity, and healthcare costs. This article aims to provide a comprehensive review of the therapeutic potential of medicinal plant extracts in the treatment of wounds infected with antibiotic-resistant bacteria. It explores the mechanisms of action through which phytochemicals exert their antimicrobial effects, such as disruption of bacterial cell walls, inhibition of quorum sensing, and interference with biofilm formation. Additionally, the review addresses the clinical relevance of these findings, highlighting promising candidates that have advanced to preclinical or clinical evaluation. Furthermore, the challenges and limitations associated with the standardization, formulation, and regulatory approval of plant-based therapies are discussed, emphasizing the need for multidisciplinary research to integrate these alternative treatments into mainstream clinical practice.
